Life cover nominations

Download the beneficiary nomination form for your life assurance, upload your completed copy, and download it again later.

On for most organisationsMost organisations have this from their first day on the platform.

If your employer provides Group Life Assurance, its page in the app explains it and asks you to keep your beneficiary nomination up to date. The nomination form is where you say who you would like to benefit. You can download the blank form, fill it in, and upload the completed copy from the same page, so a copy is on file and you can download it again whenever you need it.

Where to find it

Benefits, then My Benefits, then open your Group Life Assurance policy. The form controls are on that policy's page.

Fill in and upload your form

Open your Group Life Assurance policy and select Download blank form. This is the form your HR team has uploaded for that policy.

Fill it in and save the completed form as a PDF.

Back on the same page, choose your completed file with the upload button. It must be a PDF of 10 MB or less.

When it has gone through you see "Your completed form was uploaded". The page refreshes and shows a link to your copy.

If the file is not a PDF, or is larger than 10 MB, you see a message straight away and nothing is uploaded. Save it again as a smaller PDF and try once more.

Download your completed form

Once you have uploaded a form, the policy page shows Download your completed form. Use it whenever you want to check what you sent.

You may also see one of these messages on the page.

  • "You haven't uploaded a completed form yet" means there is nothing on file for this policy.
  • "Your completed form is on file, but a download link isn't available right now" means your form is on file but the link could not be made at that moment. Try again later.

Good to know

  • The form controls work on your own membership only. A link to a policy page that is not yours takes you back to Benefits.
  • Your HR team may also send you a task in your Inbox asking you to complete the form. The steps above are how you do it.
  • If you are a member of more than one life policy, each one has its own page and its own form.
